Important Note

This test is used to determine if a patient has primary hyperaldosteronism.

Performing Laboratory

Specimen Requirements

Procedure:

1. Patient should arrive at 8AM and be allowed to lay down and rest for 30 minutes prior to the test.

2. While patinet is still lying down at about 9 AM obtain baseline labs (Aldosterone, Renin, Cortisol, Potatssium), and baseline vitals (blood pressure, heart rate, respiratory rate)

3. Place IV and give 0.9% Normal saline 500 cc/hr for hour hours (about 2 liters total).  Patient needs to be NPO this entire time.  Patient should stay laying down except if needs to go to the bathroom.  Ok for patient to read or watch TV

4.  Check vital signs every hour.

5.  After patient has recived 2 liters of IV fluid and 4 hours after IV start, obtain the same labs (Aldosterone, Renin, Cortisol, and Potassium)  This is the most important part of the test.

This test must be performed at the Infusion Therapy Clinic.

Reference Values

Aldosterone: <or = 40ng/dL for a patient in the supine position

Postinfusion aldosterone levels great than 10 ng/dL indicates a positive test.  Results less than 10 ng/dL indiacates a negative test.  Resutls between 5 ng/dL and 10 ng/dL are indeterminate.

Monday through Friday.  Must be scheduled with Infustion Therapy Clinic.
